Hammad Shah, RPh
Chief Executive Officer
As CEO of Clarest Health, Hammad Shah, RPh, is at the helm of the organization’s vision to embrace modern technology and Personalized Medication Management to ADVANCE PATIENT HEALTH AT EVERY STEP. With an intense focus on patients, he is charting the path for the future of pharmacy care, increasing access to better, more efficient solutions.
A pharmacist by trade and education with over 30 years of experience, Hammad’s expertise crosses disciplines, including product development, sales, process improvement, and mergers and acquisitions. Before joining Clarest, Hammad focused on corporate direction and strategy as the President and CEO of SOC Telemed, successfully growing the company’s ability to connect patients to the highest quality clinicians at the right time – regardless of proximity. Previously, Hammad championed FreseniusRX’s vision to change the practice of pharmacy from simply dispensing medications to playing an integral role in patient care as their President.
Hammad has a deep understanding of operating in rapidly changing healthcare markets. His efforts and expertise in healthcare leadership have positively impacted patients and providers by increasing access to higher-quality care and services.

In my years of practicing pharmacy and personal experiences, I began envisioning an integrated pharmacy solution that begins and ends with the patient. Every decision at Clarest, no matter how small or large, starts with the question, “how will this impact the patient?” Our team works daily towards the goal of Advancing Patient Health at Every Step, and they are bound to face challenges in creating the solution that will improve lives. I am confident we will achieve that goal and overcome the obstacles by always doing what is right for the patient. Integrity is at the core of who we are as an organization, and I could not be prouder of that.

Kathleen Chagnon
Chief Legal and Administrative Officer
Kathleen Chagnon brings to her role as Chief Legal and Administrative Officer over thirty years of sophisticated legal, compliance and risk management experience. In that role, Ms. Chagnon is responsible for Clarest’s legal and compliance strategies and policies—including corporate governance, legal compliance and ethics, corporate finance activities, acquisition and new venture activities, regulatory matters, and litigation.
Previously serving as the Chief Legal and Administrative Officer of Remedi SeniorCare, Ms. Chagnon’s healthcare expertise and knowledge of M&A, corporate law and regulatory compliance, from both an in-house perspective and as a law firm partner, allow her to serve as a strategic advisor to the CEO and senior executive team in defining and implementing the Company’s significant business and legal initiatives. She has held senior executive positions in Fortune 100 companies including: SVP, General Counsel, and Chief Compliance Officer of Constellation Energy; Chief Compliance Officer and Deputy General Counsel of AIG; and VP, Corporate Group General Counsel of The St. Paul Companies. In addition, Ms. Chagnon served as a partner at DLA Piper and Saul Ewing and as an associate at Hogan Lovells. She earned her B.A. from Stanford University with honors and received her J.D. from Columbia University.

Joseph DiCarlo
Chief People Officer
Joseph DiCarlo is the CPO for Clarest and is responsible for setting and executing the organization’s overall people strategy: ensuring the organization hires, retains, develops, and engages the best talent possible in the communities in which Clarest operates.
Before joining Clarest, Joe served as Chief People Officer at Health Monitor Network, the leading patient engagement company in the United States. With an education in Political Science and Business Administration, Joe’s dynamic leadership style and extraordinary vision have garnered many national best places to Work awards for multiple organizations. He knows his commitment to building a mission-driven team, guided by core values, will result in an engaged, forward-thinking associate culture providing an extraordinary patient experience.

Jason Cohen
Chief Technology Officer
As the CTO at Clarest, Jason Cohen expertly leads crucial software engineering and technology product development to enhance Personalized Medication Management for our patients and clients continually.
Jason has mastered the art of navigating complexity and seamlessly transforming digital landscapes into cohesive ecosystems through his extensive career spanning over three decades. Before joining Clarest, he served as a distinguished Senior IT Executive at The Omnicom Group, catering to over 5,000 global clients. He successfully spearheaded more than 100 complex integrated technology integrations across various continents. Jason’s vast expertise and education guide Clarest in leveraging technology, personalized medication data analytics, and the latest generative AI solutions to positively impact patients’ lives. His visionary approach ensures that Clarest remains at the forefront of embracing modern technological advancements and optimizing personalized Medication Management.

Mark Schroder, RPh
President and Chief Operating Officer
Mark Schroder, RPh, is the Chief Operating Officer at Clarest Health. He joined Clarest Health in 2024 and brings over 20 years of experience in the long-term care pharmacy industry. Prior to joining Clarest, Mark served as Executive Vice President and Chief Operating Officer at Remedi SeniorCare, where he led the company’s operations and growth strategies.
Before Remedi SeniorCare, Mark was a co-founder and principal of Panoptic Solutions, LLC, focusing on consulting and analytic services for skilled nursing facilities. He also held various positions at Omnicare, Inc., including Vice President of Pharmacy Operations, managing multi-pharmacy operations across several states.
A pharmacist by training and a graduate of the University of Kentucky, Mark is known for his operational expertise and leadership in the LTC pharmacy sector. His background and experience support Clarest Health’s commitment to efficient operations and exceptional customer service.

Tom McCormick
Chief Commercial Officer
As Chief Commercial Officer at Clarest Health, Tom McCormick leads the commercial strategy, driving growth and expanding key partnerships.
With extensive leadership experience in healthcare, AI, and digital transformation, Tom has scaled sales teams and led market expansion at Optum, Syllable AI, Clarify Health, Sutherland Global, and Xerox. He has built long-term partnerships with government and healthcare organizations worldwide, advising leaders on healthcare interoperability and digital transformation.
A recognized industry expert, Tom has presented at global conferences and briefed policymakers in Europe and the Middle East, including Australia’s former Prime Minister Malcolm Turnbull, on best practices for digital healthcare. His expertise plays a vital role in Clarest Health’s commitment to revolutionizing Personalized Medication Management.

John Gould
Chief Financial Officer
As Chief Financial Officer of Clarest Health, John Gould oversees the company’s financial strategy, risk management, and operational performance. With more than 25 years of experience in finance and operations within the long-term care industry, he brings deep expertise in financial planning, regulatory compliance, and driving business growth.
Before joining Clarest Health, John served as CFO of Remedi SeniorCare, where he led financial and administrative functions, ensuring operational efficiency and strategic development. Prior to that, he was the CFO of the Long-Term Care Pharmacy Division at Omnicare, where he managed financial reporting, budgeting, and acquisition integration. John began his career at Deloitte as a Senior Auditor, working with public and privately held companies across various industries.
John holds a Bachelor of Science in Business Administration in Accounting from Bowling Green State University.
